4. Online Drinker's Check-Up. You can get a brief screening for free, but I don't see a way to look at the full evaluation without paying. Here is a paper describing outcomes obtained from the online checkup.
5. The Project MATCH MET manual contains a model for using assessment feedback as a motivational tool. It cites the DrInC and the Form 90 (also available in the Project MATCH series) as well as other measures.
