Motivational Interviewing, Second Edition: Preparing People for Change (MI2) by William R. Miller and Stephen Rollnick A briefer, more practice-oriented text focusing on application in medical settings: Health Behavior Change: A Guide for Practitioners (HBC) by Stephen Rollnick, Pip Mason, and Chris Butler
For application with mentally ill/dual diagnosis populations, see: Intergrated Treatment for Dual Disorders: A Guide to Effective Practice by Kim T. Mueser, Robert E. Drake, Douglas L. Noordsy and Lindsey Fox - outlines a collaborative approach to working with clients with substance abuse and severe mental illness. Includes numerous reproducible forms and handouts. Also see: Improving Treatment Compliance: Counseling and Systems Strategies for Substance Abuse and Dual Disorders by Dennis C. Daley and Allan Zuckoff - about half of this book is devoted to motivational approaches to improving treatment compliance. Includes examples of brief interventions appropriate for various stages of treatment engagement and key transition points in treatment.
For more detailed background information, see: A Meta-Analysis of Motivational Interviewing Outcome Trials - November 2004 (online slide presentation; or, download the Powerpoint file) Toward a Theory of Motivational Interviewing - November 2004 (online slide presentation; or, download the Powerpoint file) Miller/Rollnick/Moyers professional training video series, 1998 (aka "New Mexico" video series) - the examples I showed are all from this series.
Motivational Interviewing Web Page Stephen Rollnick's practice-oriented discussion board Overview of the Transtheoretical (Stages-of-Change) Model from the URI Cancer Prevention Research Center Clinical Forms and Instruments Decisional Balance, Change Plan Worksheet, Values Card Sort, and other materials may be found near the bottom of the "Library" page on the MI website. Also on the "Library" page, you will also find a link to the Motivational Interviewing Treatment Integrity measure (MITI). See how they rate "empathy" and "spirit" as an aid in self-supervision (ignore the behavior counts). All of the substance abuse treatment manuals listed on my resources page include exercises for eliciting change talk. The newly published materials describing the "MI Sandwich" assessment session may be found here. They are geared toward supervisors training front-line staff.
